Dear All

 

 

This week we had an High Potential Incident when crane topple over while negotiating an steep bend while  travelling on a graded road. The operator safely exited the cabin without any injuries. Investigation is underway and more details will be shared .

 

The common incident trend we see in the first two weeks of the new year is related to Line of Fire related to Dropped Objects and lifting. The events have all been Fail Safe due to effective ¿Zone Management¿. Lets us not forget the last LTI we had in Wells which was related to Line of fire  where one of our colleague was injured when he was in the Line of Fire of a Dropped Object( First alert attached). Please ensure to use Musta¿ed Effectively to identify potential Line of Fire situations in any activity and ensure personnel are away in case of any energy release.

 

CCTV Trend ¿ January 2025

Dear All,

 

This week¿s learning from the Safety Support Center highlights trending focus areas, particularly on LSR/LOF-Lifting/Hoist, Slips & Falls and Manual Handling. A total of 321 safe & unsafe interventions have been recorded.

 

Please use the CCTV dashboard to further analyze specific behaviors within your units/teams. Addressing and correcting unsafe behavior is a key focus area for the second week of January 2025.

Learning From Assurance

The self assurance theme (level 3 auditing) for the month of January is Environmental ManagementPlease ensure at least one level 3 Self Assurance (site visit) audit is completed by the Team Lead / SWE for each Cluster before the end of the month and is recorded in IDEAL: IDEAL (pdo.om)

 

Attached is an LFA on Forklift Pipe clamp during tubing handling.

The absence of pipe clamps on forklifts used in tubular handling activities increases the risk of pipes inadvertently rolling off, potentially resulting in DROPS incidents.
