Dear All,

 

We continue to observe incidents where personnel are in the Line of Fire (LOF) during energy release events¿situations that could have been prevented. Please ensure that your crew takes the time to identify LOF scenarios before starting any task. Utilize Musta¿ed and other planning tools to aid in identifying and mitigating LOF risks.

 

Let¿s focus on proactively identifying LOF situations that could harm our colleagues, rather than spending time investigating what went wrong after an incident.

Weekly Learning From SSC-

Dear All,

 

This week¿s learning from the Safety Support Center highlights trending focus areas, particularly on LSR/LOF-Lifting/Hoist, Slips & Falls and Manual Handling. A total of 418 safe & unsafe interventions have been recorded.

 

Please use the CCTV dashboard to further analyze specific behaviors within your units/teams. Addressing and correcting unsafe behavior is a key focus area for the second week of December 2024.

Learning From Assurance

The self assurance theme (level 3 auditing) for the month of December is Execution & SOP. Please ensure at least one level 3 Self Assurance (site visit) audit is completed by the Team Lead / SWE for each Cluster before the end of the month and is recorded in IDEAL: IDEAL (pdo.om)

 

Attached is a PWR on Road Safety ¿ Man Lost.

It is important as team on site to know what to do when a person is reported as `Man lost¿.

It is also important to know how you can plan & anticipate ahead (procedures, systems, equipment, people, drills).

PR-1287 provides the Man Lost Drill Procedure and Drill frequency.

 

Ask yourself for your own unit:

  • When was the last Man Lost Drill conducted? What was the level of quality/outcome/learnings/any gaps?
  • What will be the level of preparedness when it comes to the real scenario?
  • Is everyone on site aware of their own Roles & Responsibilities for this scenario?
  • Are the resources on site sufficient to execute Man Lost? Or do you need any support/improvement?
  • Is the Communication protocol and rescue capability adequate? (procedures, equipment, radio, vehicles)
