Although the number of incidents have reduced this year, there is yet a high percentage of Fail lucky incidents including HiPo¿s . We should not relay on luck to keep us safe but we need to ensure our controls are robust to either prevent the incident or to fail safely in case an incident happens . Historically Q4 tends to see a higher number of High potential incidents due to various factors. Therefore , let us ensure the effectiveness of our controls by utilizing the existing tools to achieve our Goal of ¿ Q4 ¿ Target ZERO¿
Please engage with your team using the UWD townhall pack and the feedback received during the same on the theme of ¿ Q4-Target Zero¿ and how can we achieve it together.
On the other hand , the Environment Authority have recently conducted site inspections at our facility, during which non-compliance with chemical storage and waste management regulations was observed. We would like your kind support to ensure full compliance to all environmental standards. Attached lessons learned are for your reference and to be discussed among the site team.

Please use the reflective learning as part of the Process Safety Day material to engage your teams at site with the intent of having an open reflective dialogue around Process Safety at your site.

- Management of Change,
- Barrier Management &
- Strength of Controls
It is essential that the conversations that take place reflect your own place of work, so please use your current WPS Risks, Bow Tie, Barrier Schematics and your last MOC to focus the conversation.

Please use the CCTV dashboard to further analyze specific behaviors within your units/teams. Addressing and correcting unsafe behavior is a key focus area for the first week of November 2024.

Learning From Assurance |
The self assurance theme (level 3 auditing) for the month of November is Planning & MOC. Please ensure at least one level 3 Self Assurance (site visit) audit is completed by the Team Lead / SWE for each Cluster before the end of the month and is recorded in IDEAL: IDEAL (pdo.om). Attached WIG pack is developed for your reference.
During recent assurance activity, one of the gap identified is that the training requirement for site electrician is not always clear. Attached PWR(Electrical Training) will give you summarized guide. Please refer to SP -1112, SP-1109, SP-2311, SP-1110,GU-861 for details.
