While incidents have decreased this year, we are still experiencing a significant number of high-potential incidents HiPos. Relying on luck for safety is unacceptable; we must reinforce our controls to prevent incidents or ensure safe outcomes if they occur. Historically, Q4 sees an uptick in HiPos due to various factors. Let¿s strengthen our controls to achieve our goal of "Q4 - Target ZERO" by enhancing the effectiveness of our existing HSE tools.

CCTV Trend ¿ September 2024 |
The top 5 focus areas from CCTV observations for the month of September are LSR/Line of fire- Lifting/Hoisting, Slip & Fall, Hands & Fingers, LSR/Line of fire-Potential Dropped Objects and Manual Handling. Please use the CCTV dashboard to deep dive into the specific behaviors related to your units/team. Arresting unsafe behavior is one of the main focus areas for 2024. |

Moreover, the attachments there are additional materials from corporate that can be used use. You can customize the materials for your operations. |
Learning From Assurance |
The self assurance theme (level 3 auditing) for the month of October is Permit to Work & Hazard Management. Please ensure at least one level 3 Self Assurance (site visit) audit is completed by the Team Lead / SWE for each Cluster before the end of the month and is recorded in IDEAL: IDEAL (pdo.om).
Attached is a PWR on the Fail Safe versus Fail Lucky concept.
As was also highlighted in last week¿s UWD Townhall ¿Q4 ¿ Target Zero¿ session, we see an increasing trend in HIPO¿s / Dropped Objects recently. Therefore adequate Zone Management is absolutely critical to ensure everyone gets home safely at the end of his/her shift. Ensure you have strong Barriers in place. Do not compromise.
