Dear All,

 

Let¿s continue engaging with our crew regarding the current incident trends. We¿ve observed a higher percentage of Fail Lucky incidents in 2024 (65% vs 35%), so it¿s crucial to ensure the right controls are in place to keep everyone away from the line of fire and ensure we fail safely.

Here are some open questions to consider during your Musta¿ed Engagements to ensure we fail safe:

  • How will you respond if the plan changes or if there is a change that impacts the plan?
  • What controls are in place for those working outside our line of sight?
  • How will we respond if we experience an equipment failure?
  • Where are the line of fire risks involved in the job? Are we staying clear of these risks?
  • What is the worst-case scenario while carrying out this activity? How can we manage this scenario?
  • When do we STOP the job? When do we intervene?

When planning our jobs, let¿s consider what could go wrong and how our crew will remain safe if an incident occurs. Focus on asking these open questions during our engagements and planning activities.

Let¿s stay proactive and work together to make this quarter our safest yet.

Weekly Learning From SSC

Dear All,

 

This week¿s learning from the Safety Support Center highlights trending focus areas, particularly on Slips & Falls, and LSR/LOF - Lifting/Hoisting. A total of 172 unsafe interventions have been recorded.

Please use the CCTV dashboard to further analyze specific behaviors within your units/teams. Addressing and correcting unsafe behavior is a key focus area for the first week of October 2024.

Learning From Assurance

The self assurance theme (level 3 auditing) for the month of October is Permit to Work & Hazard Management. Please ensure at least one level 3 Self Assurance (site visit) audit is completed by the Team Lead / SWE for each Cluster before the end of the month and is recorded in IDEAL: IDEAL (pdo.om)

 

Attached is a PWR on the Fail Safe versus Fail Lucky concept.

As was also highlighted in last week¿s UWD Townhall ¿Q4 ¿ Target Zero¿ session, we see an increasing trend in HIPO¿s / Dropped Objects recently. Therefore adequate Zone Management is absolutely critical to ensure everyone gets home safely at the end of his/her shift. Ensure you have strong Barriers in place. Do not compromise.
