Super Wednesday- Safety Stand Down |
Dear All,
Although the number of incidents have reduced this year we see a high percentage of Fail lucky incidents including HiPo¿s . We cannot always rely on luck to always to keep us safe but need to ensure our controls are robust to either prevent the incident or to fail safely in case an incident happens . Historically Q4 tends to see a higher number of High potential incidents due to various factors but let us ensure our controls are strong enough to achieve our Goal of ¿ Q4 ¿ Target ZERO¿ by ensuring effectiveness of our existing HSE tools.
Please engage with your team using the UWD townhall pack and the feedback received during the same on the theme of ¿ Q4-Target Zero¿ and how can we achieve it together. |

Learning From Assurance |
The self assurance theme (level 3 auditing) for the month of September is Pressure Control Equipment. Please ensure at least one level 3 Self Assurance (site visit) audit is completed by the Team Lead / SWE for each Cluster before the end of the month and is recorded in IDEAL: IDEAL (pdo.om).
Attached is a PWR on Zone Management Barrier requirements.
As was also highlighted in this week¿s UWD Townhall ¿Q4 ¿ Target Zero¿ session, we see an increasing trend in HIPO¿s / Dropped Objects recently. Therefore adequate Zone Management is absolutely critical to ensure everyone gets home safely at the end of his/her shift. Ensure you have strong Barriers in place. Do not compromise.
