Dear All,

 

Last week we had 2 Nearmiss incidents related to LOF/Drop, one classifies as HIPO. Below is a brief description of each event:

 

  1. Nearmiss, LOF/Drop: On 04th August 2024 around 16:04, During POOH 5 7/8 milling BHA operation and after pulled out 39 stands of 5¿ drill pipe, the derrickman was holding the stand on his hands to rack back and during opening the lock finger of the monkey board by his right foot, suddenly the safety boot stuck between diving board and lock finger. Then he tried to lift his foot which resulted the foot came out from the safety boot. After that, the safety boot dropped from the monkey board to the seat back area on the rig floor. It dropped from around 26 m height and the weight of Safety boot is around 0.5 kg. No personal injury or equipment damaged reported.
  2. Nearmiss, LOF/Drop: On 7th Aug 2024 @ 02:15 am, after completed logging a cross required interval & while pulling out wireline armored cable where it's cleaned by air using SLB W/L wiper C-plate weighted 5 kg & secured by long sling with also long chain. Suddenly due to accumulated of thick, muddy solids below the plate that caused stuck of wiper C-plate with logging cable while pulling out which has been lifted 3 meters from rotary table & later due to tension of the c-plate with the secured sling causing the c-plate slipped and resting on rotary table at same original position.

 Please plan for an engagements with your crews and discuss / verify the following:

  • Highlight the current trend and how you can play a role in reversing the trend.
  • Participate in Musta¿ed engagement and ensure LOF situations are identified, and controls are in place.

Referring to the recent Environment Authority (EA) findings and observations related to waste management at different locations within PDO concession areas, kindly find the attached pack addressing key issues, and required corrective actions.

Reminder I HSE Contractor Awards

Dear Colleagues,

 

As part of PDO¿s efforts to recognise excellence and to encourage exceptional performance in the field of Health, Safety and Environment (HSE) within our Contractor Community. We are pleased to launch the PDO HSE Contractor Awards highlighting PDO¿s key Contractors who strive towards cultivating a safer working environment.

The Awards are designed to focus on demonstrated best practices in safety as well as in areas such as occupational health, mental health, welfare, net zero, environmental sustainability and lastly to recognise efforts of key HSE Professionals in our community.

Furthermore, this award extends to recognise the HSE Professionals that demonstrate outstanding achievement in the occupational safety and health (OSH) field while also advancing their HSE capabilities and contributions towards the community.

Effective Learning:

Please ensure to implement and upload evidence of the action¿s learnings generated from the PDO Safety Day Session & DOIRC Effective learning.

Safety Stand-Down:

Please continue for the `Safety Stand-Down¿ pack attached, to serve as an aid in having reflective conversations within your teams. We encourage all sites to conduct the safety stand-down. Please ensure the same is conducted for all shifts.

 

Expectations are as below :-

  1. TLs\Operation Managers to have a stand-down with their units supervisors (Site and offices) to discuss the town hall material and have effective engagement on current worrying trend.
  2. Supervisors to engage with site crews and have the same discussion (cover 4 shifts).
  3. Supervisors to share DROP prevention CIs to UWZ team. Best ideas will be recognized.
  4. TL/Operation Managers to personalize the slides for their units if needed.
  5. TLs\Operation Managers to ensure all units are covered within 4 weeks.

 

Learning From Assurance

 

The Self Assurance theme (level 3 auditing) for the month of August is HSE MS & Safety CaseAttached is the `What is Good¿ guidance for the HSE MS & Safety Case self assurance question set, for reference when performing the audit.

 

Please ensure one level 3 Self Assurance (site visit) audit is completed by the Team Lead / SWE for each Cluster before the end of the month and is recorded in IDEAL: IDEAL (pdo.om).  Please contact UWA if any support is required, we are happy to join you on the site visits to provide coaching/guidance on the L3 assurance process.

 

An SOP has been developed to provide the process steps for completing Level 3 Self assurance using the IDEAL tool to create the audit plan and report the observations ¿ this can be accessed via this link: SOP 3295
