Dear All,

Recently we had three incidents resulting in personnel injuries related to hands and fingers being caught in the line of fire, as well as a slip and fall incident. Below is a brief description of each event:

  1. MTC / Hands & Fingers: On 29 July 2024, at 21:00, a mechanic working on a centrifugal pump experienced a serious injury. While tightening a body bolt that was stuck, he applied pressure with a spanner while his other hand was positioned between the spanner and the pump. The pump cover shifted, causing the bolt to loosen unexpectedly. As a result, the spanner slipped, and his finger was pinched.
  2. FAC / Hands & Fingers: On 30 July 2024, at around 11:30, a third-party ESP crew was rigging up for RIH ESP completion assembly. At 13:18, while guiding the control line with the shroud and motor, a crew member's glove was caught between the motor body and the control line, resulting in a painful pinch injury to his left ring finger.
  3. Waiting for classification / Slip & Fall: On 03 August 2024, during hoist rig-up operations, a floor man was tightening a turnbuckle on a guy-line while standing on a table 0.6 meters high. He lost his balance and fell to the ground.

We have seen an alarming rise in recordable incidents related to Hands & Fingers and Slip & Fall. Although we made significant progress in reversing this trend in last months, these recent incidents are a stark reminder that we must remain vigilant and proactive.

Engage with your crew and discuss / verify the following:

  • Highlight the current trend and how you can play a role in reversing the trend.
  • Participate in Musta¿ed engagement and ensure LOF situations are identified and controls are in place.
