Dear All,
We have seen an increase in incidents related to Hands & fingers, LOF/Drops and Road Safety this week. Most of these incidents occurred on the rig floor during routine maintenance activities. All these events can be easily prevented if we done an effective Musta¿ed Engagement.
A comparison of compliance with Musta¿ed Elements from the incidents that happened last year and this year is given below. We have seen the compliance to ¿Final Check¿ prior starting the job going down and an improvement in the communication element. One of the areas where we have low compliance is ¿ Control Energy ¿ and majority of our incidents are related to ¿Line of Fire¿. Please engage with your crew and discuss how compliance with various Musta¿ed Elements can be increased.

Also attached are the Safety tips for preventing DROP & Forklift incident learning video, please discuss the same with your crew and verify what controls are in place to prevent similar incidents in your unit.

CCTV Trend ¿ July 2024 |
The top 5 focus areas from CCTV observations for the month of July are LSR/Line of fire- Lifting/Hoisting, Slip & Fall, Hands & Fingers, Manual Handling and LSR/Line of fire-Potential Dropped Objects. Please use the CCTV dashboard to deep dive into the specific behaviors related to your units/team. Arresting unsafe behavior is one of the main focus areas for 2024.

Safety Stand-Down:
Please continue for the `Safety Stand-Down¿ pack attached, to serve as an aid in having reflective conversations within your teams. We encourage all sites to conduct the safety stand-down. Please ensure the same is conducted for all shifts.
Expectations are as below :-
- TLs\Operation Managers to have a stand-down with their units supervisors (Site and offices) to discuss the town hall material and have effective engagement on current worrying trend.
- Supervisors to engage with site crews and have the same discussion (cover 4 shifts).
- Supervisors to share DROP prevention CIs to UWZ team. Best ideas will be recognized.
- TL/Operation Managers to personalize the slides for their units if needed.
- TLs\Operation Managers to ensure all units are covered within 4 weeks.

Learning From Assurance |
The Self Assurance theme (level 3 auditing) for the month of August is HSE MS & Safety Case. Attached is the `What is Good¿ guidance for the HSE MS & Safety Case self assurance question set, for reference when performing the audit.
Please ensure one level 3 Self Assurance (site visit) audit is completed by the Team Lead / SWE for each Cluster before the end of the month and is recorded in IDEAL: IDEAL (pdo.om). Please contact UWA if any support is required, we are happy to join you on the site visits to provide coaching/guidance on the L3 assurance process.
