Dear All,

 

We continue with our standdown across the fleet to address the Line of Fire management(LOF) across our fleet.  Use the attached engagement pack to engage with your crew to raise the awareness on the trend and how we can jointly address this.

Also attached is the HSE flash of incidents presented last week,  have an engagement with your teams on how effective implementation of Musta¿ed in planning this activity could have helped in preventing LOF  related injuries  and others incidents.

This week we have seen an increase in incidents related to tubular dropping off the catwalk while pick up / laydown. Please ensure the catwalk areas in your location is having proper zone management around catwalks  to prevent personnel from accessing these areas while catwalk is in operation.

¿       Engage with your crew on the current trend using attached pack

¿       Discuss how you can prevent similar incidents on your location

¿       Participate in Musta¿ed engagement and provide feedback to respective Ops team

¿       Escalate any support needed in effective implementation of Musta¿ed

 

Our focus area for the week based on CCTV behaviors is Line of Fire LOF\Lifting & Hoisting. You are advised to plan your interventions accordingly and generate effective discussion accordingly so we can proactively stop undesired events

The expectation within Wells is that this is a commitment across departments and throughout the organization, both within PDO and all of its contractor community

 

Please use the reflective learning as part of the PS Day material to engage your teams at site with the intent of having an open reflective dialogue around Process Safety at your site.

Please use your own material to reflect on the following key themes:

    • Risk Normalization,
    • Management of Change,
    • Barrier Management &
    • Strength of Controls

 

It is essential that the conversations that take place reflect your own place of work, so please use your current WPS Risks, Bow Tie, Barrier Schematics and your last MOC to focus the conversation.

Let us continue with the focusing on Process Safety Day 2023 ¿ Commitment to Process Safety via the campaign we kicked off . By utilizing the attached pack , we request you to continue engagement sessions with the site crews with the aim of arresting the HSE trend and ensuring safe operations. E-CCC Effective learning ¿UWD Process Safety Day 2023¿ is now updated and 30th of November is put as a deadline for all site engagements verification.
