Dear All

 

Last week we had a trend of High Potential Incidents that are under LOF/Drops & stored Energy themes. The total number of HIPO¿s incidents in this year is 28 including 10 DROP incidents. The scenario of the incidents as following:

 

  • (HPE) Equipment Asset / LOF Drops: Operation was POOH 6 1/8¿ BHA, driller handover the brake to AD, the AD continues POOH operation and while he was picking up the third stand of 4¿ HWDP, the traveling block hit the mast crown block bumper assembly. After the collision, Driller attends the floor and start lowering the string, after 1 m observed a drop of bumper rubber part (~8-10 Kg, 27 m height) to rig floor. Red zone was properly managed and no personal injury, Operation suspended and recovery on going.
  • (HIPO) Nearmiss / Stored Energy: the crew were picking up DP from the power catwalk and the D.M was filling the tank using the line connected to the nearby dehydrated water flow line. Suddenly, rupture noise sounded in location and the power catwalk stopped working. The crew from the Rig floor observed the water was flowing from the dehydrated water flow line connection and then they stopped the water. Observed the clamp connecting the floe line elbow to the hoist supply line flew 34 m and hit the HPU control box, causing the hydraulic catwalk to trip & stop operating.

 

Kindly make sure the Musta¿ed 7 steps are implemented effectively.

 

Stand Down

We will continue regarding the stand down for a high potential incident classified as AIPSM Tier 1, which occurred on Tuesday, 28 November 2023 at approximately 9:04 am. The incident involved a release of hydrocarbon gas due to Electrical Overhead line construction activities occurred in the Fahud area. A 16¿¿ Gas pipeline from Fahud B to Fahud F Station was damaged. Fortunately, there was no harm to people and the pipeline was successfully isolated, repaired and brought back to service.  

We have had similar incidents in Wells previously (refer attached pack) . Please use the attached pack to engage with your teams including contractors/subcontractors. Please utilize this pack to have an open and constructive dialogue, discuss possible causes of this incident, agree and implement suitable immediate actions to prevent the reoccurrence of a similar event. 

Expectations:

  1. Work stand-down to take place immediately across all sites and to be repeated for other shifts to ensure all staff are covered including those on rest days.
  2. Both PDO and Contractors¿ management to lead the engagements and to follow up on the progress/agreed actions.
  3. Contract holders to follow up with their respective contractors on the status of the work stand-down.
