Dear All,

 

We have  seen an increasing trend of injurious incidents and High potential incidents quarter on quarter. Last week we had High Potential Incidents related to release of stored energy resulting in injury to a floorman. The effective implementation of Musta¿ed in planning this activity could have helped in preventing this incident. Brief description of the incident is as follows.

  • Incident with consequences Stored Energy during Cementing pressure test:  At 00:02 hrs on 29/10/2023 Rig  was in the process of preparing for cementing job of 9.5/8¿ casing .The quick latch cementing head was installed by the crew of cementing contractor after several attempts as the latching mechanism was not engaging correctly.  Safety line was installed on the high-pressure pumping line and the assistant cementing operator signaled the Driller that the installation is ready for circulation. Subsequently the Driller started the mud pump for  circulation at the rate of 0.5 m3/min and PP:916 KPa. Suddenly the cementing head assembly ejected from the casing coupling, hit the casing collar while landing and then, hit the floorman who was standing near the pumping line.

 

To address the above trend we are initiating a standdown across our fleet. Use the attached engagement pack to engage with your crew to raise the awareness on the trend and how we can jointly address this.

    • Strength of Controls

 

It is essential that the conversations that take place reflect your own place of work, so please use your current WPS Risks, Bow Tie, Barrier Schematics and your last MOC to focus the conversation.

Learning from Assurance: Completeness of Equipment Inspection Reports

 

Equipment (MPI) Inspection reports are sometimes being accepted as evidence ¿good for service¿, without a thorough examination report attached/in place.

The attached PWR provides a high-level summary of the requirements for a good quality Inspection report.

 

Using Musta¿ed STEP-2:  CHECK EQUIPMENT.

 

Please ask yourself:

  • How do you know a Lifting & Hoisting inspection report (for example MPI) is satisfactory?
  • Where do you look for on the inspection report itself?
  • What is the quality of the report? Is it acceptable?
