Dear All,

 

Hands & Fingers will continue to be our focus for this week. Looking to the recent Lost Time Injures  ,  it was noticed that 5 out 7 LTIS were caused due people positioning their hands and fingers at the crush points during performing the task. As well , there were  clear indications of poor planning and weak intervention culture.

Waste Management

 

As per  SP-2194 for waste management, all pits to be lined and no oil discharge to pits. Waste to be properly segregated onsite with each skip (Green for recyclable waste, Orange hazardous waste, Yellow non-hazardous, Black domestic food waste), also Grey Metal scrap.

  1. Yellow Skip: Non-hazardous and Non-recyclable Waste.
  2. Black skip: Food Waste.
  3. Green Skip: Recyclable Waste.
  4. Orange Skip: Shall be used for hazardous.

UWA Learning From Assurance - Permit to Work Guide

Dear All,

 

Given the recent incidents, this week¿s Learning From Assurance continues the theme on prevention of Hand & Finger injuries.

 

The attached Practice Worth Replicating Poster on Hands Free Tools.  Please review the following key questions:

 

  • Is the Contractor Hands Free Policy available and implemented?
  • Identify the hazards involved.  Do you have a complete Pinch Point Register and are the correct Hands-Free Tools identified or the hazards Engineered out?
  • Are all the required Hands-Free Tools identified available on the rig site (and not locked in a store)?
    • Push/Pull Sticks
    • Stabbing Guides/tools
    • Tangle Free Tag Lines
    • Torquing Tools to prevent the use of Flogging Spanners
    • Any other identified Hands-Free Tools
  • Are crews aware of the correct use of Hands-Free Tools?
