Dear All,

 

Last week, we had a High Potential Incident fortunately without any consequences to people. On 11th July 23, at approximately 14:10 hrs, the operation was RIH fishing assembly on stands.  The crew stopped to adjust the power tong & tighten the chain for slips.  At this time, the Derrickman (DM) decided to take a rest in the shaded area on the monkey board.  In doing so he removed the self restraint & fall arrester from the monkey board and connected the crown fall arrester to his safety harness. He then went to the shaded area on the monkey board and laid down to rest.  When the Assistant Driller (AD) started to RIH the fall arrester line caught the elevator handle.  The DM reacted and tried to free the fall arrester line but was unsuccessful, resulting in him being pulled down from the monkey board, hanging by his safety harness. Derrickman has been sent to clinic for preventive check-up, no injury sustained. Please utilize the attached file which is re presenting some of the previous incidents related to monkey board and working on high to have an interactive discussion with the site crews.

 

Hands & Fingers

Hands & Fingers will continue to be our focus for this week. Looking to our statistics ,  Hands & Fingers injuries constitutes 41% of the Total Recordable Case (TRC).  These incidents caused due people positioning their hands and fingers at the crush points during performing the task. As well , there were clear indications of poor planning and weak intervention culture.

UWA Learning From Assurance - Safety Critical Equipment

Dear All,

 

Recent HSE Banding assessments have highlighted that there is some ongoing misunderstanding of the requirements on how to identify and manage Safety Critical Equipment.  This week¿s Learning From Assurance is a revisit of an LFA to address a common issue that has also been identified in recent level 1 & 2 audits.  The attached LFA indicates some of the considerations for managing SCE.  Last month a workshop was also held by UWA to bring together our Contractors to address the expectations and requirements for managing SCE.

 

Please review the attached and the following key questions:

 

  • Is SCE identified for your unit?
  • Has the SCE been identified from the Major Accident Hazard (MAH) bowties developed in the HSE Case?
  • Is the SCE clearly identified in the Maintenance System (PMS) to ensure it receives suitable focus?
  • Are there Performance Standards for all SCE to ensure the barrier is effective?
  • Is there a process to ensure an MOC is in place for all `defeated¿ barriers (ie those that do not meet the Performance Standard, or have overdue maintenance)?
  • How is SCE management compliance monitored (both on site and by management)?
  • Is a Safety Critical Spares list available and managed (based on the SCE identified in the bowties)?  How often is this verified to ensure all spares are available?
