Dear All,

 

Last week, we suffered from multiple injurious incidents related to Hands & Fingers and one of them resulted into an unfortunate Lost Time Injury (LTI) where IP¿s finger was sadly amputated. Out of the 7 LTIs that we suffered in UWD so far, 5 of them are related to Hands & Fingers. Moreover, Hands & Fingers constitutes 41% of our Total Recordable Case (TRC) injuries by Jun-2023. This is really a worrying trend which requires a serious consideration and bring the focus back in place via capitalizing on the basics. we must work together to stop hurting our people. All the incidents are avoidable.

The latest injury took place in Weatherford Rig-148, Assistant Driller was working on TDS to tighten the pull-down lines. While performing this task he got caught & pinched in his right index finger.

In addition, recently we have observed as increasing in HIPOs. Last week, we were hit by 2 HIPO¿s, fortunately, no one was injured. Therefore, your commitment to adhere to the basics and fail safe, will protect our people from being hurt.

 

UWA Learning From Assurance - Permit to Work Guide

Dear All,

 

This week¿s Learning From Assurance has followed on from last week¿s learning and shares a learning pack from the Emergency Response Level 2 audit conducted back in 2018. This audit learning was not shared at the time, except through PIM actions and reporting via MSE. The learnings attached herein are still relevant today as similar gaps exist.
