We completed the first month of the year and a visible trend is the ¿Line of Fire¿ incidents related to Dropped Objects. The incidents were fail lucky i.e. the controls were not sufficient to the keep personnel away from the dropped object and we were lucky no one was hurt.
Last week, we had another high potential incident involving a dropped object. During rig-up operations, a work floor leg was being lifted from a horizontal to a vertical position using a crane and web sling attached to a lifting clamp. However, two pins designed to secure the lifting clamp to the leg retracted from their holes, causing the clamp to detach from the leg. As a result, the leg, which was suspended by the crane and web sling, fell to the ground between the banksman and the tagline operator.
When we look into the Musta¿ed checks related to this incident, there were multiple opportunities where we could prevented the incident during the planning phase but we did not. Lets engage with the crew using attached HSE flash and discuss how can we effectively use the Musta¿ed Tool to prevent incidents / fail safely in case we have an incident.
Environment Management
Another trend we see is the issues with non-compliance to ¿ Waste Management & Chemical Storage¿ in our locations . This is leading to fines from environment authority and complaints from the locals. Let us ensure we manage our sites / camps in accordance with the environment regulations and return the location the way it was handed over to us prior carrying out the activity. During handover if you notice any non-compliance please address it or escalate it for support if it cannot be addressed.
