Super Wednesday

Dear All,

We still see dropped objects incidents at our sites especially in  Rig floor and  catwalk areas. The last drop incident occurred last week during POOH operation, while breaking out and laying down of string stabilizer and lifting sub using rig power catwalk carrier, this has resulted in lifting sub to fall down1.1 meters  on the rig floor without sustaining any injuries or equipment damage.

As you can see from the attached HSE flash there were multiple failures in our Musta¿ed process which could prevent the event. Please remind the crew that during Musta¿ed engagements to have effective planning .

Let us remind our crew that Starting the JOB is not the first step!

Moreover, there were 2 incidents involving the electric wire on junction box which got spark/fire while running the power swivel engine. The crew observed the spark and use fire extinguishers put out the fire immediately and engine stopped, remove the nearby crane, and bring mechanic and electrician to check the unit. We appreciate the crew for immediate action to control the fire.

As well, last week we had October townhall session where we showed recent HSE trends and explained the focus area for Q4. We also presented the results out of the Hands and Fingers Campaign. We would like to thank all of you for the great effort put in arresting the Hands and Finger injures trend. Attached is the townhall pack for your rollout at site.

Musta¿ed

As you can see from the initial findings from the attached HSE flashes few of the Musta¿ed steps were missing which lead to the event happening. It is important that we ask open question for each of these elements so we can have an effective engagement with the crew carrying out the job. In the coming weeks we will be sharing some of the questions you can ask to the crew carrying out the task when you do your Musta¿ed Engagements. Let¿s focus on STEP 4 this week-Control Energy

Examples of Questions you can ask during TBT/Planning for each Musta¿ed element :

STEP-4:  Control Energy

ASK: How has the equipment been isolated? Explain.

ASK: Exposure to Line of Fire: How is Energy controlled?
