Last week we had two incidents related to inadequate controls during lifting operation . In one incident the crane became imbalanced and in the other the tandem lifting was not adequately managed leading to asset damage. Proper planning of these lifts and an effective Musta¿ed engagement could have helped prevent these incidents.
Incident #1 Near miss: During preparing for Hoist Move, the crane was loading the Cement block on flatbed trailer. The weight of CMT block is 7.3 tone and the boom was extended almost 12 m, during loading, cement block was almost 1.5m above the trailer. The crane operator descended the block using boom instead of descending with the crane hook, the crane got unbalanced causing CMT block to lay down and hit the trailer, and then the crane returned to the normal position.
Incident #2 Equipment Damage: Slickline operation was in progress, crew members were lifting lubricator into rig floor. The lubricator was lifted from two sides, the top side was attached to travelling block via elevator controlled by assistant driller and the bottom side was hanging by mobile crane. The banksman was giving signal to crane operator to pull back the lubricator, the assistant driller thought the signal was for him, and he started to raise the travelling block which lead to the breaking of the lifting bracket of stuffing box.
Please engage with your crew and discuss how can similar incidents be prevented on your location. Also ensure the following on site
- Lifting activities are managed using a lift plan
- An effective Musta¿ed Engagement takes place prior to any lifting activities
- 10 question for safe lift is discussed prior to any lifting activities

Dear All,
A common UWA observation from field activities relates to the implementation of the Lifting Plan requirements. The attached PWR provides a summary of the requirements to help guide the PDO Supervisor in self-auditing the requirements. Please also refer to SP-2273. Lifts are categorized into Routine and Non-Routine lifts with specific requirements, as follows:
Routine Lifts
- Loads < 5MT, < 50% of crane/appliance capacity, repetitive lifting, normal environment, suitable conditions.
- Loads of known and evaluated weight, shape and CoG.
- Simple operations with 1 crane/lifting appliance using basic rigging arrangements.
- Generic lifting plan can support these lifts as long as the Contractor has LEC & Lifting Supervisor trained.
- The lifting plan must include the minimum requirements in SP2273 section 9.1.4 and be authorized by the Approved Appointed Person.
- The format of the Lifting Plan must be approved by UEQ.
Non-Routine Simple Lifts
- Heavier than 5MT and ¿ 20MT, and less than 75% of lifting capacity, blind lift, confined/restricted area or night/reduced visibility.
- Loads of known and evaluated weight, shape and CoG.
- Simple operations with 1 crane/lifting appliance with a lift point directly above the load.
- Generic or Specific Lift Plans approved by the PDO Lifting Engineer.
- A specific risk assessment , PTW and pre-use inspection are minimum requirements.
Non-Routine Complex Lifts
- Loads over 75% of the crane/appliance capacity and > 20MT, 2 or more lifting appliances, transferring load between lifting appliances or personnel lifting.
- Lifting with restricted headroom, near overhead power lines or over live HC equipment (eg wells, flowlines)
- Specific lifting planning (lift plan, Engineering document, rigging plan and dedicated risk assessment with checklist) approved by PDO Lifting Engineer.
- Prepare a work pack including a method statement and drawings in conjunction with an SME
- PTW is required and the lift to be completed by an experienced supervisor and operator.
