Dear All,

 

We have had 3 repeat incidents since we started the year one related to tubular handling (having the wrong elevator), one related to load securing and one related to dropping of tool string while pulling out the tool. Two of these events were fail safe as there were controls in place to keep personnel away from the line of fire.

 

The key learnings are related to inadequate planning (not checking the equipment) &  inadequate communication / handover between the operators. Please ensure your site start implementing ¿Musta¿ed¿ to ensure they transition to work safely and controls are maintained throughout the job.  

 

Tubular Handling:

 

To prevent dropped objects from tubular handling activities on the rig /hoist floor please ensure the following

 

  • For activities involving elevators to lift or hoist tubular, a pre-activity check must be conducted and include (but not limited to) physical verification through measurement of internal diameter (ID) of the elevator versus outside diameter (OD) of the tubular.
  • A visual inspection of the elevator must be carried our prior to use to ensure the physical integrity of the elevator
  • A workplace checklist with respect to safe handling of tubular SHALL be used and applied for tubular handling operations

MUSTA¿ED : Are you ready?

 

We have started coaching PDO & contractor personnel to be ambassadors of Musta¿ed who in turn will coach our field staff on what is expected from our frontline crew on Musta¿ed. We will be holding more engagement sessions across various levels of the organization to create awareness on Musta¿ed.

 

  • What is Musta¿ed?
  • A structured approach to ensure 100% readiness before commencing any task through 7 simple steps in order to complete that task safely.
  • Reason for change
  • More than 70% of incidents having Planning, inadequate equipment / procedure etc as underlying cause
  • Musta¿ed helps in proper transitioning to work by following 7 simple steps
  • Focus on frontline crew
  • Complements our current safety leadership initiative FLS 1 & 2
  • Not a new initiative but a structured way of working

Learning From Assurance

 

This week¿s Learning From Assurance is on Man Lost response.  Please find attached PWR 8.7.3 `Road Safety & Man Lost¿.

 

It is essential that a detailed man lost procedure is available and understood by key personnel.  Man lost drills must be regularly performed to ensure Journey Managers are fully aware of their roles and the procedure is understood.  The drills also ensure IVMS data is accessible and emergency contacts are accessible.   

 

Timely and efficient response to a Lost Man scenario is vital.

 

Each site shall ensure:

  1. The Man Lost procedure is available and understood by Journey Managers.
  2. Man Lost drills are performed at least once a year for each Journey Manager.
  3. All drivers are aware of the importance of call timings and adhering to the route on the JMP.
  4. IVMS data is accessible.
  5. The reporting/communication protocol is understood and numbers are correct.
  6. Verify emergency team response is timely.
  7. All lessons learned from Man Lost drills/events are logged and the procedures updated as required.
