Dear all,

Last week we had a High potential incident when a wireline crew member while driving to do a site survey lost his way and got stuck in the sand. He had to walk more than 20 kms to reach the nearest unit and man lost emergency response had to be activated. Investigation is progressing and more will be shared. The initial findings show us that there were compliance issues with Journey management and  Emergency response process which led to this incident.

The summer months are starting and we need to prepare our sites and onboard the crew to address this change in weather. Please ensure your sites start implementing the summer mitigation measures and initiate the process to obtain the summer working waivers from ministry.

Let us continue with the safety standdown initiated across UWD units to reverse our current incident trend. Please ensure all your crew have attended the engagement and escalate any support needed or dilemmas identified.

Effective Learning tool- New learning uploaded

A new action  learning related to ¿Flange Makeup¿ has been uploaded in the effective learning tool as learning from various incidents related to flanges not sealing effectively and failing prematurely causing a loss of primary containment. Please acknowledge the learning and upload evidence to close the actions related to the learning. The time line for closing this learning is 30/06/2022.

Road Safety

The new Road Safety Standard ( SP 2000 Ver 5) has been rolled out and the go live date is July 2022. The rollout pack & the key changes between the old and new standard can be downloaded from UWD HSE website.

Reporting Road defects

We have seen an increase in events related  to bad conditions of the road, road related signages etc. To ensure you get a quick response please ensure the issue is raised to the following focal point from PDO with the details of the locations ( GPS location) and the specific issue is highlighted in the email .
